We recommend using the store as all data transmission is encrypted via 128-bit SSL (Secure Socket Layer) protocols whereas fax and voice transmissions over standard telephone connections are NOT encrypted. We recommend ordering through our secure cart system as opposed to email in order to provide the highest level of security regarding your personal information. However, if you do need to place an order by email, please follow these instructions: - Download the order form as Text (*.txt) in compressed ZIP format or view in your browser and save as a text file.
- If the form was downloaded in ZIP format, extract and complete the form. If you saved the form from browser view, simply complete the form.
